To ensure your application can be fully assessed, you must:
• Upload all relevant supporting documents
• Provide proof of income for all household members aged 18 and over
Please submit one of the following for each adult in your household:
• A Notice of Assessment from your most recent income tax return (please black out all information except your name, address, effective date, and income on Line 15000), or
• A recent notice or cheque stub from an assistance or benefits program.
