Complete and submit this form to receive an income qualification code. This is an optional step which allows us to review your proof of income in advance.

Proof of income must be submitted for each member of your household over the age of 18. The income qualification code will be valid for 6-months and can be used to apply for any FortisBC income-qualified rebates. The code does not guarantee rebate approval. All rebate terms and conditions must be met.

This can be a Notice of Assessment from a current income tax return (black out all information except for your name, address, effective date and income on Line 15000) or a notice or cheque stub from an assistance or benefits program.
